Insurance Information

We accept most group health plans and are happy to file claims with your insurance company as a courtesy. Important note: Please remember to bring your insurance card and a photo ID with you to each appointment. If you don't bring your insurance card, we will request payment in full for your visit. If you forget your photo ID, you may have to reschedule your appointment. Federal laws against identity theft now require us to verify our patients' identities.
